You know the eight reportable incident types. So why do the late notifications, the missed notifications and the compliance findings keep coming? Because SIRS is not decided in the clear cases. It is decided in the grey: the resident-to-resident push, the expected death that was not, the refusal of care that looks like dignity of risk on Monday and neglect by Friday.

This masterclass takes managers and clinical leads past the definitions and into the judgment calls, and shows you how to put a defensible written rationale behind every call you make.

You leave with the SIRS Decision Trail Record, a worked root cause analysis you can adapt, and four grey-zone case scenarios with facilitator answer guides, ready to re-run with your own leadership team.

What the masterclass covers

Three parts across 90 minutes, built on cases, triage drills and group scenarios.

The eight types, worked through the grey
Every reportable incident type, including the newly redefined neglect, taken to the calls that most often go wrong: consent and cognitive impairment, cumulative minor incidents, restraint versus treatment.
Priority 1 versus Priority 2, in practice
The current Priority 1 definition and its practitioner-treatment test, the triage workflow, when the notification clocks start, and who owns the decision out of hours.
The defensible decision trail
What survives an audit two years later: contemporaneous records, reasoning written against the statutory definition, and the same rigour applied to the decisions not to report.
